PSW Represented at Hill Briefing on APMs
PSW CEO Melanie Matthews was invited to speak on the panel for a Hill Briefing on Alternative Payment Models (APMs). Melanie joined other healthcare leaders to discuss real-world examples of successful APMs and the benefits that these models can provide to patients. Specifically, Melanie focused on bundled payment models and the opportunities these models present to better incorporate specialty providers into accountable care.

Celebrating Certified Nursing Assistant Week
PSW recently had a chance to celebrate our nursing assistants during Certified Nursing Assistant Week! This dedicated team helps countless patients and their hard work deserves to be recognized. PSW is proud to have such a wonderful team.
Patient Success Story
A 105-year-old bundle program patient was discharged from the hospital with her daughter and was scheduled to receive home health services. When PSW reached out to provide Care Management services, the daughter expressed concern that her mother had stopped eating 24 hours ago. She considered going to the ER because the home health agency was not responding.
The PSW Care Navigator discussed options with the daughter, who then requested a hospice consult. The Care Navigator coordinated with home health and arranged for hospice to visit the next morning. The patient was admitted to hospice services that day.
A few days later, the PSW Care Navigator followed up and found that the daughter was very satisfied with hospice care, and her mother was comfortable at home with her needs met.
