FOUNDATIONAL CARE MANAGEMENT SERVICES
Complex Care Management (CCM)
- The goal of CCM is chronic disease management and to teach patients chronic disease self-care.
- The services would include acute and post-acute follow-up care and complete care coordination.
Transitional Care Management (TCM)
- The goal of TCM is to reduce inpatient length of stay and emergency room utilization.
- The services would include post-acute management, medication review, general needs assessment, tracking changes in health status, and 30-day post-discharge patient management.
