An Accountable Care Organization (ACO) is a healthcare delivery model that involves a group of healthcare providers and facilities working together to provide coordinated care to a defined population of patients. The goal of an ACO is to find an innovative way to provide high-quality care to beneficiaries while lowering costs and generating savings to the Medicare Trust Fund. This is achieved by coordinating the care for ACO beneficiaries to ensure that they get the right care at the right time. ACOs are designed to shift the focus of healthcare delivery from volume-based care (fee-for-service) to value-based care.

Our Experience
In 2016, PSW launched its ACO, NW Momentum Health Partners (NWMHP). A formidable effort, accelerated by CMS changes, this pathway solidified our participation in the CMMI models and delivered a clinical, financial, and administrative return to these providers. Today we have 3 ACOs under the PSW umbrella and provide administrative services to partner ACOs. PSW has experience and service contracts for the Bundled Payments for Care Improvement Advanced (BPCI-A) model, the Next Generation ACO Model, the Direct Contracting Model, the ACO REACH model, and the Medicare Shared Savings Program (MSSP).
