Jake Woods

Jake Woods is the Executive Director of Accountable Care Organizations (ACOs) at Physicians of Southwest Washington (PSW). In this role, he leads the strategic direction and governance of PSW’s ACO portfolio. Jake also oversees Marketing and Communications, business development, and policy advocacy efforts to advance PSW’s mission of transforming care for patients and providers.

Over the past several years, Jake has been at the forefront of PSW’s ACO strategy, playing a critical role in its growth and expansion across the Pacific Northwest. His leadership has helped position PSW as a regional leader in value-based care, fostering partnerships and innovative models that align incentives for better outcomes.

Jake is pursuing a Master of Healthcare Administration from the University of Minnesota School of Public Health and a Bachelor’s degree in Communications from the University of Washington. His academic foundation, combined with extensive experience in accountable care, fuels his passion for healthcare innovation and policy advocacy.
